Each license requires different documentation to fulfill statutory requirements. We assist each provider in gathering this supporting documentation. Unfortunately, many of these required documents come in different formats that are not accepted by AHCA. We help format these documents to accepted AHCA standards and submit the application on your behalf.

Required Documentation:

Home Medical Equipment Licensing

  • Commercial General Liability Insurance with $250,000 per occurrence coverage
  • Professional Liability insurance with $250,000 per occurrence coverage
  • Active and fully executed lease OR warranty deed reflecting the provider’s address
  • Local city or county zoning letter
  • Proof of Financial Ability to Operate filled out completely and reviewed by a licensed CPA

Required Documentation:

Healthcare Clinic Licensing

  • Any and all medical licenses for owners or employees of the provider
  • Provider employee list with eligible background screenings
  • Medical Director/Clinic Director Agreement
  • Proof of Financial Ability to Operate filled out completely and explained
